Beginning on 1 November 2025, older Australians now have a clearer, simpler pathway to the help they need for clinical care, independence, everyday support, and the right products and home. What is the Support at Home Program?

Support at Home is the government’s re-designed in-home aged care program that has now replaced Home Care Packages (HCP) and Short-Term Restorative Care (STRC). It introduces eight ongoing service classifications, each with a quarterly budget aligned to your assessed needs. If you’re already on an HCP, you’ll move across on a matching classification; if your needs change, you can be re-assessed.

Support at Home also adds flexible short-term pathways including a Restorative Care Pathway (typically up to 16 weeks, with additional allied health funding) and an End-of-Life Pathway (time-limited, higher funding so people can remain at home). An Assistive Technology & Home Modifications (AT-HM) scheme provides funding tiers for equipment and home modifications, to support safety and independence.

How Do I Access the Support at Home Program?

From 1 November 1, 2025, the Home Care Packages (HCP) program was retired and was replaced by the Federal Government’s new model for home care – the Support at Home (SaH) program.

Existing Home Care Packages Clients

If you were assessed on or before September 12, 2024, you are considered a “Grandfathered Client.” In terms of any out-of-pocket expenses, you will remain under the previous Home Care Package arrangements, protected by the No Worse Off Policy

If you were assessed after September 12, 2024, you will be subject to the new rules of co-payments set by the Government’s means testing. You will be transitioned to the Support at Home Program and have access to all of the service categories. You will receive a quarterly Support at Home budget and will be able to spend any remaining unspent Support at Home funding on services from the Assistive Technology-Home Modifications (AT-HM) scheme.

To access the Support at Home program, you will first require an assessment via the Single Assessment System.

How Much Funding is Available?

Support at Home uses a tiered funding structure designed to match your level of need.
If you are approved for government funding through My Aged Care, you will be placed into one of the program’s eight funding levels.

  • Individuals requiring assistance with personal care as well as routine household activities.

  • Individuals with very high and complex care needs requiring a significant level of ongoing support.
  • Individuals requiring the highest level of care due to complex, intensive or multiple support needs, which may include palliative care.

Each level represents increasing clinical and support needs, and budgets are reviewed regularly to ensure your funding aligns with your circumstances.

In addition to the core funding levels, Support at Home also provides targeted pathways and add-on funding options, including:

Restorative Care Pathway

up to 16 weeks of allied-health-focused reablement support.

higher, short-term funding to help people remain comfortably at home.

Support at Home Management Options

We offer a mix of fully managed, self-managed, and blended options, so you can make the most of your Support at Home Package.

Fully managed care means that we take the administrative headache out of care so you can focus on living well. We use our team of support workers, allied health professionals, and qualified subcontractors to coordinate everything from care planning, service scheduling, clinical oversight, budgeting, purchasing assistive technology items, and organising home modifications. You will have your own dedicated care partner who will respond to your changing needs.

Prefer more control? We also offer a self-managed option for those capable of managing their end-to-end services with our help. We will still develop your care plan and provide care management services through your dedicated care partner, but you will choose your own support workers, allied health professionals, and subcontractors and coordinate these services in accordance with the care plan. Because you are managing the budget, you will be required to pay for these services upfront and submit your invoice for weekly reimbursements via the easy-to-use reimbursement platform. It’s transparent, with lower service costs, so you get more out of your Support at Home Package.

Blended Managed Option

Support at Home Packages now offer a mix of both fully managed and self-managed options. 

You may choose to manage some of your services, while allowing us to manage others. Talk with us today about your specific circumstances.

101 Home Care offers high-quality services at competitive pricing. All of our support workers undergo mandatory training and compliance verifications. For Self-managed Services, the service is cost plus 10% to cover company overheads.

Support at Home Service Categories

The Support at Home program has three separate service categories, each with different means-tested co-contributions. Services in the Clinical care service type have no co-contributions for all Australians. Those in the Independence category require a co-contribution of 5% for full pensioners, up to 50% for self-funded retirees without a Commonwealth Seniors Health Card. Services in the Everyday Living category require a co-contribution of 17.5% for full pensioners, up to 80% for self-funded retirees without a Commonwealth Seniors Health Card. The new separate AT-HM fund is for purchasing equipment, aids and home modifications and has a co-contribution rate equal to the Independence category. Once you have been approved for your Support at Home Package, the Government will inform you of your co-contribution levels. Grandfathered Home Care Package (HCP) clients assessed prior to September 12, 2024, are protected by the No Worse Off Policy and often do not have a co-contribution.
