Can I Change My Support at Home or Home Care Package Provider?
Yes. Whether you are currently receiving services through the Support at Home program or have entered care through the newer Support at Home program, you may change providers at any time.
The Support at Home Program Is Replacing Home Care Packages
The Australian Government is transitioning from Support at Home to the Support at Home program. During this shift:
- Existing clients may remain on an Support at Home
- Newer clients may begin under Support at Home
- All clients retain the right to transfer their care
Importantly, your funding balance moves with you, ensuring:
- No loss of unspent funds
- No disruption to essential services
- A smooth transition supported by the 101 Home Care team
Will I Keep My Current Therapists and Support Services?
Yes. Continuity of care is a priority at 101 Home Care.
If you currently work with a physiotherapist, podiatrist, occupational therapist, or any other clinician or provider, we will make every effort to maintain those existing relationships when you transition to us.
Changing providers does not mean losing the supports you value.

How to Change Support at Home / Home Care Package Providers
Switching Support at Home Providers in Three Simple Steps
1. Notify Your Current Provider
Advise your current provider in writing—via email, letter, or fax—that you wish to transfer your care. Most agreements require 2–4 weeks’ notice, depending on your service arrangement.
2. Reactivate Your Referral Code
Contact My Aged Care on 1800 200 422 and request that your referral code be reactivated. Please record this code for your new provider.
3. Contact 101 Home Care
Once you have your referral code, contact our team. We will arrange a time to visit your home, discuss your care needs, and establish your new services to ensure a seamless transition.
